Hero Banquet 2010 Nomination Form

Heroes come in all forms. Do you know of a local hero? Download, fill out and return the Hero Banquet Nomination form soon. You may nominate multiple people by filling out and submiting more than form. You may also fill out the lower portion of this form to order tickets for this unique event.
 
Nominations are now being accepted for local Quad City heroes to be celebrated at Community Caring Conference's 17th Annual Quad City Community Hero Awards Banquet on October 7th 2010. You and your neighbors are invited to nominate your hero for one or more of the following categories:

YOUTH This award is for a young person (under 18) from the community who has done something special for a friend, neighbor, classmate, parent or community ... someone who makes a difference!

EDUCATION This award honors a teacher, instructor, administrator, or a student who has made a contribution for their community or an organization, agency or individual who has an outstanding educational program, or a special project with youth or targeted groups.

ORGANIZATION This award is given to groups or organizations that offer special programs to the community that make a positive difference.

BUSINESS This award is handed to a local business that is actively involved in addressing and supporting community needs and willing to help.

LAW ENFORCEMENT This award honors any outstanding law enforcement person or agency that has made a significant difference within the community.

BI-STATE This award is for any individual, organization, agency or business that is community-minded, represents both Illinois and Iowa, and has contributed to the betterment of the Quad City area.

INDIVIDUAL This award allows individuals who are not connected to any group, but who made an outstanding contribution to the community.

CRIME FIGHTER OF THE YEAR This award recognizes individuals within the law enforcement field who made an outstanding contribution to the community.

Remember to offer your best testimonial for your nominee as there are usually many nominations to be chosen from.
